BHS Transcript Request

Directions:

 

OFFICIAL Transcripts for College Admissions are only sent directly to colleges.  In the MAIL TO NAME field, please indicate the name of the college you are applying, as well as the physical address to their admissions office.  

Please note, if you are applying via the The Common App or The Common Black App, you only need to request one transcript to complete the application regardless of how many colleges your are applying to using this application.  In the NAME field, you will indicate which App you are using and will not need to fill out the physical mailing address.

 

OFFICIAL Transcripts for Scholarships, NCAA Clearinghouse, Organizations or Employers are only sent directly to them.  In the MAIL TO NAME field, please indicate the name, as well as the physical address.  If there is an email address that it needs to be sent to instead, please indicate that in the NAME field and leave the address blank. 

 

UNOFFICIAL Transcripts for Current Students will be emailed to the student's BHS email address.  In the MAIL TO NAME field, please list your BHS email address and leave the address blank.

 

UNOFFICIAL Transcripts for Former Students will be mailed directly to you.  If you prefer to have it emailed, in the NAME field, indicate your email address.  

 

If you paid for multiple transcripts, you will have to fill a form out for each transcript (sorry).
